When a Lifetime Care Plan is purchased, you will be given an inspection card. The inspection card will need to be presented every six months along with the piece covered for cleaning and inspection. If any covered repair work needs to be done, the updated inspection card will be required.
To maintain your Lifetime Care Plan, you must have your jewelry inspected by a Helzberg Diamonds associate during the six months following the date of purchase and every six months thereafter. Inspections must be documented by a Helzberg Diamonds associate on your inspection card. If you do not, your coverage under this Lifetime Care Plan will become invalid.
Just bring your jewelry to your nearest Helzberg Diamonds store along with your Lifetime Care Plan inspection card. Your card will be updated and used to verify you have maintained the needed cleaning and inspections to keep the care plan active. All check-ups and Covered Repairs and Services are a part of the purchase price of this plan. When we have completed the inspection and any needed repairs, all you have to do is pick up the jewelry at the same store where you dropped it off.
This Lifetime Care Plan covers all inspections, cleaning and Covered Repairs and Services to the jewelry covered by this Plan. Covered Repairs and Services include the parts and labor necessary to fix problems which occur during normal usage of the jewelry, consistent with the conditions for which it was designed, and all costs covered under the original manufacturer’s warranty (except as limited by the Addenda in this Limited Care Plan and applicable state law). Depending on the type of jewelry for which the Plan coverage is purchased, it would include shortening of diamond bracelets, resizing or reshanking of rings, the replacement of lost or damaged diamonds and gemstones, restringing of pearls, clasp replacement, repair of broken chains, repair of worn or broken prongs, repair of broken earring posts or similar repairs.
Jewelry that is not brought in for its regular six-month check-up or that has been damaged as a result of excessive or abusive treatment
This Lifetime Care Plan gives you coverage for the lifetime of the jewelry from the date of purchase. This coverage applies only to the jewelry listed on this plan and is not transferable to other jewelry items purchased.
This Lifetime Care Plan applies to the original customer, the original gift recipient of the jewelry and subsequent gift recipients of the jewelry, so long as he or she is in possession of both the jewelry and the Lifetime Care Plan inspection card.
For any single claim, the limit of liability under this Plan is the least of the cost (1) authorized repairs, (2) replacement with a product of like kind and quality, (3) reimbursement for authorized repairs or replacement or (4) the purchase price you paid for the product, including appropriate sales tax. The total liability under this Plan is the purchase price you paid for the product, including appropriate sales tax. In the event that the total of all authorized repairs exceeds the purchase price paid for the product, including appropriate sales tax, we replace the product, or we reimburse you for replacement of the product or the purchase price you paid for the product, we shall have satisfied all of our obligations under this Plan.
You may cancel this Lifetime Care Plan at any time within 30 days after the date of purchase by returning the inspection card with your receipt to any Helzberg Diamonds location. Except where prohibited by applicable state law, Helzberg reserves the right to terminate or cancel this Plan in accordance with applicable state law in the event of your fraud or misrepresentation or your default under any of the terms and conditions applicable to your customer account with Helzberg or related financing
Where the covered product was initially acquired under an RTO Transaction, any cash settlement or refund will be payable to the owner of the product at the time the settlement is made. This will be the Lessor if You have not yet acquired ownership of the property. In all other respects, the Lessee will retain a beneficial interest in this Plan and all non-cash benefits described herein shall be rendered to the Lessee. Any cash settlement or refund paid to the Lessor will be applied to reduce Your obligations under the RTO Transaction. Any amount in excess of the balance due to purchase the item under the RTO Transaction will be payable to the Lessee by the Lessor. Any owner obligations related to maintenance of the product shall be the responsibility of the Lessee during the term of any RTO Transaction except as provided by law. Any reference to purchased, sold, or similar terms shall include “leased” and its derivatives. Any reference to purchaser shall mean the Lessee under the RTO Transaction and not the Lessor. This Section will not apply unless the Lessor is indicated on the sales receipt provided at claim time.
The above constitutes the entire terms and conditions of this Lifetime Care Plan and no such terms or conditions may be amended without the mutual written agreement of both parties. Helzberg Diamonds’ obligations under this Plan are backed by its full faith and credit. The attached receipt held in the slip portion of the service contract contains the service contract number and product description, and is hereby incorporated by reference. We are glad the associate listed on the receipt was able to assist you in making this responsible choice. The obligor, or service contract provider, is Helzberg’s Diamond Shops, Inc. California/Oregon Addendum
This Lifetime Care Plan is guaranteed by Berkshire Hathaway. Helzberg's Diamond Shops, Inc. is a 100% owned subsidiary of Berkshire Hathaway. Berkshire Hathaway agrees to guarantee all obligations of Helzberg Diamonds relating to the operation of a care plan in your state during such time as Berkshire maintains ownership of Helzberg. This guarantee is irrevocable as long as there is in force in your state any contract or any obligation arising from Helzberg service contracts sold while Berkshire owned Helzberg. Florida Addendum
The rate charged for this contract is not subject to regulation by the Florida Office of Insurance Regulation. In the event the contract is cancelled by the warranty holder, return of premium shall be based upon 90% of unearned pro-rata premium less any claims that have been paid or less the cost of repairs made on behalf of the warranty holder. In the event the contact is cancelled by Helzberg Diamonds, return of premium shall be based upon 100% of unearned pro-rata premium less any claims paid or the cost of repairs made on behalf of the warranty holder.
Georgia Addendum
If this Lifetime Care Plan is cancelled within the first thirty (30) days you will receive a 100% refund. If you cancel after thirty (30) days of receipt of your contract, you will receive a pro-rata refund of the contract price. A time period of 7 years will be used to calculate a pro-rata refund. In the event of cancellation by Helzberg Diamonds, notice of such cancellation will be in writing and given at least 30 days prior to cancellation. Cancellation will comply with Section 33-24-44 of the Code of Georgia. Claims paid and cancellation fees shall not be deducted from any refund owed as a result of cancellation by Helzberg or you. Helzberg may not cancel this contract except for fraud, misrepresentation, or non-payment by you. Any refund owed and not paid as required is subject to a penalty of 25% of the refund owed and interest of 18% per year until paid; however, such penalty shall not exceed 50% of the amount of the refund. This is not a contract of insurance. Obligations of the obligor under this contract are insured by a bond with Hartford Fire Insurance Company. If you file a claim under this contract and the obligor fails to pay or provide service within thirty (30) days of such filing such claim, or if you are otherwise dissatisfied, please submit your claim in writing and a copy of this contract and the sales receipt for the product to The Hartford Fidelity & Bonding (BOND) Hartford Plaza, 690 Asylum Avenue, Hartford, CT 06115
Illinois, Arizona, Nevada and California Addendum
For Illinois, Arizona, California and Nevada customers only, this Lifetime Care Plan may be cancelled at any time during its term, subject to the following conditions: If this Lifetime Care Plan is cancelled within the first thirty (30) days of the coverage period and so long as you return the Lifetime Care Plan Kit with your receipt, Helzberg Diamonds will refund the entire purchase price (100%). If this Plan is cancelled at any time thereafter and so long as you return the Lifetime Care Plan Kit with your receipt, Helzberg Diamonds will refund to you a pro-rata portion of the purchase price based on the following calculation. A time period of 7 years will be used to calculate a pro-rata refund. To calculate the month in which coverage began under this Lifetime Care Plan, it will be considered a month only if this Lifetime Care Plan was purchased before the 15th of such month. The month in which this plan was cancelled will be considered a covered month only if it is cancelled after the 15th of such month. Pro-rata refunds will be given based on the number of months remaining in the extended warranty period, divided by 84, multiplied by the original selling price. For example, a $179.99 plan purchased on January 7, 2008, and returned on July 7, 2008, would have 78 months remaining out of 84, so the calculation would be 78/84 x $179.99, to arrive at a refund amount of $167.13.
Missouri Addendum
If the care plan is cancelled within the first (30) days of the coverage period (or within 10 days of the receipt of the contract, whichever happens last), Helzberg Diamonds will refund the entire purchase price (100%) of the plan. A ten percent (10%)penalty shall be added to a refund that is not paid or credited within forty-five(45)days of return of the contract to the provider. This contract is not an insurance contract.
New Hampshire

New York
For New York, this Plan does not cover the replacement or repair of chipped gemstones.
South Carolina Addendum
For any disputed claims, you may contact: South Carolina Department of Insurance Capital Center, 1201 Main Street, Suite 1000 Columbia, SC 29201 (803) 737-6180. A ten percent (10%) penalty per month shall be added to a refund that is not paid or credited within forty-five (45) days after the return of the contract to the provider.
Vermont Addendum
Rent to Own (RTO) is not applicable for this state.
Wisconsin Addendum
THIS CONTRACT IS SUBJECT TO LIMITED REGULATION BY THE OFFICE OF THE INSURANCE COMMISSIONER OF INSURANCE.
Helzberg Diamonds may cancel this Lifetime Care Plan only for your nonpayment of the provider fee, your material misrepresentation to Helzberg Diamonds, or your substantial breach of duties relating to the covered product or its use.
If Helzberg Diamonds cancels this Lifetime Care Plan, you will receive a written notice by mail at your last-known address contained in Helzberg Diamonds’ records. You will receive the notice at least 5 days prior to cancellation. The notice will state the effective date of the cancellation and the reason for the cancellation.
If Helzberg Diamonds cancels this Lifetime Care Plan for a reason other than nonpayment of the provider fee, you will receive a 100% refund of the unearned pro-rata provider fee, less any claims paid.
If you cancel this Lifetime Care Plan within the first 30 days of coverage, the Lifetime Care Plan will be void and Helzberg Diamonds will refund to you the full purchase price of this Lifetime Care Plan, so long as you return the Lifetime Care Plan Kit with your receipt and no claim has been made under this Lifetime Care Plan. The right to void this Lifetime Care Plan is not transferable and will apply only to the original Lifetime Care Plan holder.
If you cancel this Lifetime Care Plan after the first 30 days of coverage, or if you cancel within the first 30 days but a claim was made under this Lifetime Care Plan during the first 30 days, Helzberg Diamonds will refund to you 100% of the unearned pro-rata provider fee, less any claims paid. Helzberg will charge an administrative fee of 10% of the provider fee for such cancellations.
In the event of a total loss of the property covered by this Lifetime Care Plan that is not covered by a replacement of the property under this Lifetime Care Plan, you will be entitled to cancel this Lifetime Warranty Plan and Helzberg will refund to you 100% of the unearned pro-rata provider fee, less any claims paid.
Pro-rata refunds will be calculated using a time period of 7 years. If Helzberg Diamonds fails to pay or credit a refund within 45 days after the return of the Lifetime Care Plan as described above, Helzberg Diamonds will pay a 10% per month penalty of the refund amount outstanding which will be added to the amount of the refund.
When you purchase a watch from us, we guarantee that it is free of any manufacturing defects. Should you decide your new watch doesn't meet your needs, return it to us within 30 days of purchase, unworn, complete with the original packaging, manufacturer's warranty materials, any watch band links removed during sizing, and your sales receipt. We'll help you choose another watch to suit your needs, or refund your money.
Unlike other jewelry, watches contain delicate machinery and electronics. The manufacturer guarantees your watch to be free from manufacturing defects. A description of the repairs covered by the manufacturer of your watch are detailed in the enclosed warranty. A 3-year watch extended care plan can be purchased and will cover damage to or defects with the crown, stem, crystal, band or clasp. The watch extended care plan does not cover minor scratches and nicks caused by normal wear and tear but does allow for a one-time cosmetic refurbishing.
